From Exceptional OPS:
The Fayette County Health Department and the Georgia Department of Health, District 4, is conducting a survey relating to Community Health (health problems, risky behaviors and quality of life). Our organization has been participating in this ongoing assessment. We are forwarding it to our members and friends for their participation.
To access the survey, please go to: https://www.surveymonkey.com/s/FayetteNeedsAssessment
If you believe access to health information and services for people with Developmental Disabilities is a problem in Fayette County and could use improvement, please note it in the “Other” portion in Part 1, Question #2
This is for Fayette County residents only. You can only take the online survey once. The last day to participate in the online survey is 23 June. Other surveys are being conducted for other counties in Georgia.
Thank you for your assistance and participation in this effort!
